Chicago's Bistro 110 received a favorable writeup in Time Magazine in March 2005:  "Food for allergic customers are to be prepared on uncontaminate surfaces and hand-deliverd by the chef."
Maggiano's in Cincinnati and Indianapolis serves gluten free pasta.  Call ahead for reservations and advise them you will need gluten free pasta.  This is an upscale Italian restaurant, prices are $12-$20, service is great as is the atmosphere.  Worth a stop.  Cincinnati's resaturant is located just off I-71, near exit 12, near the Kenwood Mall.  Maggiano's also has locations in a number of states - including Indiana (Indy), Tennessee (Nashville), Illinois, and Florida.
Danatos in Cincinnati serves pizza without a crust.  It is for those on the Atkins diet, but works well for those on a GF diet.  The pizza is a one person serving.
Cheeseburger In Paradise has a gluten free menu.

Check out the page called "Other Support Groups" web sites.  Many of those sites also list restaurants local to their area.  If you are headed on a trip, it might be smart to check their information.  You just have to be diligent and look!
Cornerstone Cafe And Bakehouse in West Chester, Ohio outside of Cincinnati serves a GF pizza.  This information came from a celiac support group member in Cincinnati.   April 2008.  Best to search the web for its location.  Worth the trip if you are in the Cincinnati area.
